Nearly a month after checking into rehab, Nick Stahl has checked out ... of his own volition, according to one source.
"He left a week ago," a source told E! News. "He walked out against doctors' advice. He just didn't want to be there anymore...It is very sad. He's still struggling and has yet to hit rock bottom. Everyone is extremely worried about him."

Stahl was spotted in downtown Los Angeles a few days ago, the source said. It is unclear where he is now. Stahl's wife did not return phone calls from E! News seeking comment.
News of Stahl's rehab check-out comes after the Terminator star went missing in May and checked into rehab a week later.
Prior to heading to the treatment facility, Stahl sent an e-mail to friends apologizing for "worrying everyone," adding he wanted to get help and was planning to seek treatment.
