Many of us misunderstand what ?long-term care? means. Some people feel that it is the end of a person?s rights and the end of life. There are many definitions of long-term care. The one that works the best is the definition from our own country?s regulations:

Nursing home residents have rights and certain protections under the law. The nursing home must list and give all new residents a copy of these rights.

Resident rights include:

  • Respect: You have the right to be treated with dignity and respect.
  • Medical Care: You have the right to be informed about your medical condition, medications, and to see you own doctor. You also have the right to refuse medications and treatments, or choose services.

What about if I am at the end of life?? What if I choose hospice care? What if I want to live in my nursing home?? Most people can choose to stay in their nursing home and choose hospice care.

What is hospice care??Hospice offers care to help individuals with progressive illnesses live each day to the fullest. Hospice can assist people with physical, spiritual, emotional, and social needs. The focus of hospice care is to maintain the dignity and comfort of patients while offering support to the family. Hospice does not speed up or slow down the dying process. Hospice views death as a normal occurrence in the continuum of life and gives support and expertise during the natural dying process.

Choosing hospice care is not giving up.? Hospice does have different goals than other health care providers: palliative care rather than curative care. It is a decision to maximize quality of life by managing symptoms and controlling pain. If the patient?s condition improves, or the patient decides to seek curative treatment, he or she can leave the program. If the patient needs hospice services again, they can re-enroll.

Sutureless method for joining blood vessels invented

ScienceDaily (Aug. 29, 2011) ? Reconnecting severed blood vessels is mostly done the same way today -- with sutures -- as it was 100 years ago, when the French surgeon Alexis Carrel won a Nobel Prize for advancing the technique. Now, a team of researchers at the Stanford University School of Medicine has developed a sutureless method that appears to be a faster, safer and easier alternative.

In animal studies, a team led by Stanford microsurgeon Geoffrey Gurtner, MD, used a poloxamer gel and bioadhesive rather than a needle and thread to join together blood vessels, a procedure called vascular anastomosis. Results of the research are published online Aug. 28 in Nature Medicine. Lead authors of the study were Stanford postdoctoral scholar Edward Chang, MD, and surgery resident Michael Galvez, MD.

The big drawback of sutures is that they are difficult to use on blood vessels less than 1 millimeter wide. Gurtner began thinking about alternatives to sutures about a decade ago. "Back in 2002, I was chief of microsurgery at Bellevue in New York City, and we had an infant -- 10 to 12 months old -- who had a finger amputated by the spinning wheel of an indoor exercise bike," said Gurtner, senior author of the study and professor of surgery. "We struggled with reattaching the digit because the blood vessels were so small -- maybe half a millimeter. The surgery took more than five hours, and at the end we were only able to get in three sutures.

"Everything turned out OK in that case," he continued. "But what struck me was how the whole paradigm of sewing with a needle and thread kind of falls apart at that level of smallness."

Sutures are troublesome in other ways, too. They can lead to complications, such as intimal hyperplasia, in which cells respond to the trauma of the needle and thread by proliferating on the inside wall of the blood vessel, causing it to narrow at that point. This increases the risk of a blood clot getting stuck and obstructing blood flow. In addition, sutures may trigger an immune response, leading to inflamed tissue that also increases the risk of a blockage.

The new method could sidestep these problems. "Ultimately, this has the potential to improve patient care by decreasing amputations, strokes and heart attacks while reducing health-care costs," the authors write in the study.

Earlier in his career, as Gurtner contemplated a better way of joining together blood vessels, he considered whether ice could be used to fill the lumen, the inner space of the blood vessel, to keep both ends open to their full diameter long enough to glue them together. Not feasible, he concluded. "Water turns to ice quite slowly and you would have to drop the temperature of the surgical site a lot -- from 98.6 degrees to 32 degrees Fahrenheit," he said.

Shortly after arriving at Stanford in 2005, Gurtner approached fellow faculty member Gerald Fuller, PhD, professor of chemical engineering and the Fletcher Jones II Professor in the School of Engineering, about whether he knew of a substance that could be turned easily from a liquid to a solid and back to a liquid again, and that would also be safe to use in vascular surgery. Fuller immediately suggested a Food and Drug Administration-approved thermoreversible poloxamer called Poloxamer 407. It is constructed of polymer blocks whose properties can be reversed by heating.

Fuller teamed up with Jayakumar Rajadas, PhD, director of the Stanford Biomaterials and Advanced Drug Delivery Laboratory, to modify the poloxamer so that it would become solid and elastic when heated above body temperature but dissolve harmlessly into the bloodstream when cooled. The poloxamer then was used to distend both openings of a severed blood vessel, allowing researchers to glue them together precisely.

The researchers used a simple halogen lamp to heat the gel. In tests on animals, the technique was found to be five times faster than the traditional hand-sewn method, according to the study. It also resulted in considerably less inflammation and scarring after two years. The method even worked on extremely slim blood vessels -- those only 0.2 mm wide -- which would have been too tiny and delicate for sutures. "That's where it really shines," Gurtner said.

Dermabond, a surgical sealant, was used to attach the ends of the blood vessels together.

Poloxamers have been used before as a vehicle for delivering drugs, including chemotherapeutics, vaccines and anti-viral therapies. Researchers have used Poloxamer 407 to occlude blood vessels in experimental animals for the purpose of evaluating the gel's safety and efficacy in so-called "beating heart surgery," in which certain vessels need to be temporarily blocked to improve visibility for the surgeons performing a coronary artery bypass.

Although other sutureless methods have been developed, they generally have not produced better outcomes, the authors said. "Often, the use of microclips, staples or magnets is itself traumatic to blood vessels leading to failure rates comparable to or higher than sutured anastomoses," they wrote.

"This is a novel approach to anastomosis that could play a valuable role in microvascular surgery," said Frank Sellke, MD, chief of cardiothoracic surgery at Brown University Medical Center and associate editor of the Journal of Thoracic and Cardiovascular Surgery, who was not involved in the study. "But it really needs to show that it holds up in clinical trials."

The authors say further testing on large animals is needed before human trials can begin, but they note that all of the components used in the technique are already approved by the FDA. "This technology has the potential to progress rapidly from the 'bench to bedside,'" they write.

Gurtner said he believes the new technique could satisfy a huge unmet need and prove especially useful in minimally invasive surgeries, in which manipulating sutures takes on a whole new level of difficulty.

Michael Longaker, MD, the Deane P. and Louise Mitchell Professor in the School of Medicine and a co-author of the study, called the technique a "potential game-changer."

"When you're bringing together hollow tubes, whether they're large structures, like the colon or the aorta, or a small structure, like a vein in the finger of a child, you're always worried about lining them up directly and effectively sealing them," Longaker said. "The technique that Dr. Gurtner has pioneered could allow surgeons to perform anastomosis more quickly and with improved precision."

He continued: "Coming up with this solution was the result of the classic Stanford model of bringing together researchers from a variety of disciplines."

Other Stanford co-authors of the study were postdoctoral scholars Jason Glotzbach, MD, Kristin-Maria Sommer, PhD, Oscar Abilez, MD, PhD, and Cynthia Hamou, MD; medical student Samyra El-ftesi; and technician Travis Rappleye.

The work was supported by a Stanford Bio-X Interdisciplinary Initiatives Research Award and the Oak Foundation. Stanford University has patented the technology.

Gurtner and Longaker are also members of the Stanford Cancer Institute.

Ex-NBA player Crittenton arrested in murder case (AP)

LOS ANGELES ? Former NBA player Javaris Crittenton was in police custody after being arrested at a Southern California airport to board a flight to Atlanta, where his lawyer said he would go to surrender in a murder case.

Crittenton, who was suspended from the league for having guns in a locker room with teammate Gilbert Arenas, was arrested Monday evening at John Wayne Airport in Orange County after checking in for a redeye Delta Air Lines flight, FBI spokeswoman Laura Eimiller said. She said he was with a male companion who was not arrested.

Crittenton was taken to a Los Angeles police station where he was booked on suspicion of being a fugitive from justice. Eimiller said he will be held pending a possible court appearance in state court Wednesday before being taken to Atlanta.

Crittenton, 23, was wanted on a federal arrest warrant after the FBI learned that he traveled on a one-way ticket to Los Angeles, where he was known to have family and friends, several days after a deadly Aug. 19 shooting.

Crittenton's attorney told the Associated Press earlier Monday that his client planned to surrender. He said he was in contact with Fulton County District Attorney Paul Howard and the FBI Fugitive Task Force to make arrangements for Crittenton's arrest.

"He offered to turn himself in," lawyer Brian Steel said. "He's not guilty. We look forward to getting it to the courts."

Eimiller said she wasn't aware of any negotiation for Crittenton to surrender.

Police in Atlanta had obtained a murder warrant for Crittenton in the Aug. 19 shooting of 22-year-old Jullian Jones, a mother of four young children, as she was walking with two men. Police believe one of the men with her was the target.

Police spokesman Carlos Campos had said the motive appears to be retaliation for a robbery in April, in which Crittenton was a victim.

When he was with the Washington Wizards in December 2009, Crittenton and Arenas had a dispute over a card game on a team flight. Two days later, Arenas brought four guns to the locker room and set them in front of Crittenton's locker with a sign telling him to "PICK 1." Crittenton then took out his own gun.

Crittenton pleaded guilty in January 2010 to a misdemeanor gun charge and received a year of unsupervised probation. Arenas entered his guilty plea on Jan. 15. He served a short time in a halfway house.

Crittenton is on the roster of the NBA developmental league's Dakota Wizards.

Eminem showed no fear last year at the MTV Video Music Awards, nabbing Best Male Video for "Not Afraid" over Drake, Usher and B.o.B. But in 2011, fearless, lie-loving Eminem was no match for the feverish smile of Justin Bieber.

The Biebs took home the win for "U Smile," winning out over Em's "Love the Way You Lie," Cee Lo Green's "F--- You," Bruno Mars' "Grenade" and Kanye West's "All of the Lights."

After presenter Kim Kardashian announced his name, Bieber popped up, snagged a smooch with girlfriend Selena Gomez, and headed up onstage, where he delivered a heartfelt message about the role of a higher power in his ascendance to the top of the young pop crop.

"I just want to say thank you so much, not only to God but to Jesus," Bieber said. "Because I wouldn't be here without Him. He's really blessed me. He's put me in this position. So I want to say thank you so much."

The win marked two Moonmen in two years for Bieber, who nabbed Best New Artist for "Baby" at the 2010 VMAs (besting Ke$ha, Nicki Minaj and Jason Derülo) and who is quickly making certain his name and music are deeply engrained in the MTV award show. Last year, of course, Bieber opened the VMAs with a medley outside in Nokia Plaza. This time around, he skipped a performance but still found himself at the center of the action inside the theater.

"Also, I want to say thank you to all the other nominees," Bieber said. "They were amazing. I'm a true fan of all of them, so I want to share this award with all them. You guys are amazing."

He started his UFC career with a 0:40 knockout, and then celebrated with a Matrix-esque backflip off the cage. That kind of striking will make fight fans clamor for what's next.

(AP) ? Israel is tired of Hollywood filming Jesus' crucifixion in Italy and the Crusader invasion of the Holy Land in Morocco.

So Israeli officials are promising better tax breaks, terror attack insurance and handouts of up to $400,000 to lure international movie producers to the holy city of Jerusalem. They want to cash in on the multibillion-dollar industry, and want the real Jerusalem on the silver screen ? not Mediterranean stand-ins.

"It's absurd. Movies set in Jerusalem are filmed in Malta, Morocco and Greece," said Yoram Honig, an Israeli film director and 10th-generation Jerusalemite. He heads the Jerusalem Film Fund, which was set up three years ago to encourage more moviemaking in the city.

According to conventional wisdom in Hollywood, Jerusalem is too volatile to ensure smooth filming on location. International insurance companies have traditionally refused to provide terrorism risk coverage, or offered it at exorbitant prices.

For a long time, it didn't make financial sense for the producers. While Israel in the 1980s attracted such star-studded productions as Sylvester Stallone's "Rambo 3" and Chuck Norris' "The Delta Force," it later lost out to other countries that started giving big tax incentives to producers.

"If they think it's expensive and dangerous, they won't want to come," Honig said.

That's why the Israeli government enacted a law in 2008 offering tax breaks to foreign film companies that choose to shoot in Israel. And earlier this year Israel introduced an insurance fund to provide coverage to a production in case of disruptions by acts of war or terrorism, said Zafrir Asas, manager of audio visual industries in Israel's Ministry of Industry, Trade and Labor.

But the 2008 law has had little effect. Asas admits the tax incentives are far lower than what other countries provide.

Nava Levin, the Israeli representative to the Producers Guild of America, said the law actually creates obstacles to filmmakers, including a requirement that Israeli production companies purchase goods and services for the producers on their behalf. The law "is written in a way that is almost impossible to take advantage of it," Levin said.

Even Israeli producers have shied away from the city: Out of about 600 Israeli movies filmed since the country's founding, only about 30 have been filmed in Jerusalem, Honig said. That has begun to change recently, with some of Israel's most celebrated new films shot here with the fund's financial support, including Joseph Cedar's "Footnote," which was awarded best screenplay at this year's Cannes film festival.

Now the city is sweetening the pot for international filmmakers, offering cash incentives and a municipal department that will assist with filming permits and on-location logistics. Only four international productions are shot in Jerusalem each year, most of them European, Honig said.

Part of the push to get Jerusalem into movie theaters is to present a more positive image of the city than the conflict seen in the news ? "the Jerusalem that more than 3.5 billion people of faith around the world wish to see," said Stephan Miller, spokesman for Jerusalem Mayor Nir Barkat.

Honig said the municipal fund is close to signing a contract with a German producer to shoot a film about the trial of Nazi war criminal Adolf Eichmann, which took place in Jerusalem in 1961. An Italian producer has also proposed filming a comedy in the city about an Italian nun who falls in love with an ultra-Orthodox Jew.

Other projects the film fund is courting include an Indian-Israeli romance, and "Jerusalem, I Love You," an installment of producer Emmanuel Benbihy's Cities of Love series. A delegation of Bollywood producers also recently visited the city to scout out filming opportunities. And a new animation studio in the city, Animation Lab, has courted Hollywood producers to work on its first feature film, "The Wild Bunch," slated for release in 2012.

Tel Aviv and Haifa, too, are developing similar film funds to attract producers to those cities.

In the meantime, most major Hollywood productions have preferred to set up their movies about Jerusalem elsewhere.

Take "World War Z," the forthcoming multimillion-dollar zombie flick starring Brad Pitt. Part of the plot takes place in Jerusalem, but producers have replicated the city on the island of Malta, which offers hefty cash rebates for foreign film productions. Israeli actors have been flown in for the filming, Levin said.

"Robin Hood: Prince of Thieves" opens with Kevin Costner escaping from a prison in Jerusalem ? but the movie was filmed in England and France. Mel Gibson's "The Passion of the Christ" was shot in Italy. In Steven Spielberg's "Munich," about Mossad assassinations of Palestinians who killed Israeli athletes at the 1972 Olympics, a Tel Aviv beach promenade scene was filmed in Malta.

Some films taking place in Jerusalem have even been filmed in Middle Eastern countries that don't have friendly relations with Israel. The Crusaders who storm Jerusalem in the 2005 action film "Kingdom of Heaven" were filmed in Morocco, which cut off diplomatic ties with Israel in 2000 when a Palestinian uprising erupted.

Monty Python's "Life of Brian" filmed scenes of Jerusalem in Tunisia, using part of the recreated Jerusalem set built for the Italian miniseries "Jesus of Nazareth." Tunisia only established diplomatic relations with Israel in 1996, but severed them in 2000.

The opposite is true, too. The 1991 thriller "Not Without My Daughter" starred Sally Fields as an American trapped in Iran, but it was filmed partially in Israel. The opening scene of the 1999 film "The Insider," when Al Pacino's character meets the founder of the militant group Hezbollah in Lebanon, was actually filmed in an Arab village in Israel.

Galaxy and DROID Prime May Challenge iPhone (NewsFactor)

A couple of seriously powerful Samsung Android phones will hit the market around the same time as the iPhone 5, setting up a challenge for post-Steve Jobs Apple.

If reports are accurate, the match-up will test iOS 5, the latest version of Apple's mobile operating system, running on the latest iPhone, against the latest version of Google's Android operating system, 4.0, code-named Ice Cream Sandwich, running on a 4G device by Samsung.

Making things even more interesting, the Korean electronics giant is engaged in a legal battle with Cupertino, Calif.-based Apple over alleged patent infringement by Samsung.

The Samsung DROID Prime has been reported by Boy Genius Report to be the next weapon in the arsenal of Verizon Wireless. BGR says the phone will be ready for prime time in October with an exclusive on the updated Android system. Its sister phone, the 4.3-inch touchscreen, dual-core Galaxy S II, will be available on the other three networks.

The DROID Prime may be the reason Verizon passed on the Galaxy refresh, wary of too many similar, competing devices on its shelves.

It would be the second Samsung DROID device: The Charge was released for Verizon's LTE network in April. No specs on the DROID Prime have emerged, but DROID devices made exclusively for Verizon typically have large touchscreens and faster processors than many rival phones.

Samsung has confirmed that the Galaxy S II will be capable of LTE data speeds. That means it could soon be the first LTE device for AT&T, which promises to have its network available later this year. AT&T spokesman Mark Siegel refused comment on the matter.

A Korean-language version of Samsung's website promises that the phone, a version of which is already available overseas, will be an "LTE 4G market-leading smart device....The world's first LTE/WCDMA/GSM multi-mode support" phone.

The website statement, first reported by Bloomberg News, says the phone will be announced on Friday at the IFA electronics trade fair in Berlin.

Reuters reports that Samsung also will introduce ChatON, a new instant-messaging service that runs on its bada operating system as well as Android and will be pre-installed on feature and smartphones beginning in October. It also will be available for download for users of iOS and BlackBerry devices, the report said, enabling instant exchanges across a wide range of the mobile Internet.

The iPhone 5, believed to be ready for roll-out in September or October, has been said to include updates that are mostly in the operating system, although it may be slimmer and have a slightly larger screen. Many analysts do not believe that it will be capable of 4G speed.

Will that give rivals an edge?

"The 14-month old iPhone 4 still outsells any individual LTE phone at Verizon Wireless," said analyst Avi Greengart of Current Analysis. "So it's pretty clear that simply having faster network speeds does not trump the iPhone's advantages in apps, content, accessories or user interface."

But that doesn't mean speed doesn't matter.

"As long as battery life isn't overly degraded," Greengart said, "having LTE is a competitive advantage."

    On Saturday, February 21, 2009, Full Sail University (fullsail.edu) was proud to serve as the location for A Gift For Teaching?s (http://www.agiftforteaching.org ) annual fundraising event, ?Lights, Candles, Auction,? which welcomed nearly 300 guests and raised over ,000 to benefit the mission of this non-profit organization.

    Held throughout several soundstages on the Full Sail University campus, and emceed by Mitch English of the nationally syndicated morning talk show, The Daily Buzz, the evening served as a celebration of A Gift For Teaching?s ten-year birthday and commitment to providing supplies for students and teachers in the local school community. Sponsored by Bright House Networks, the event offered guests the opportunity to participate in a themed Hollywood-style evening including red carpet interviews, live music, raffles, a silent auction, and showcased Crayon Masterpieces created by local school students.

    'Bubble Boy' Kids Living Normally After Gene Therapy: Study (HealthDay)

    WEDNESDAY, Aug. 24 (HealthDay News) -- More than a dozen children with so-called "bubble boy" disease are alive and well, with functioning immune systems, nine years after undergoing gene therapy to correct their disorder, researchers report.

    Most of the patients attend school with other children, something that probably would have been fatal without treatment.

    "The promise of gene therapy is being fulfilled, at least for these diseases, where a number of patients are walking around in good health because they had gene therapy," said Dr. Donald Kohn, professor of microbiology, immunology and molecular genetics and pediatrics at the University of California, Los Angeles.

    The disorder -- severe combined immunodeficiency (SCID) -- compromises the immune system so severely that children can't fight off normally innocuous infections. The condition is rare, and the term "bubble boy" was coined after a Texas boy with the condition lived in a germ-free plastic bubble.

    Only boys inherit the gene in question, and many born with SCID die in infancy.

    Two studies published Aug. 24 in Science Translational Medicine detail the results of the gene-modifying treatment. Kohn wrote a perspective piece accompanying the studies.

    Traditionally, the only treatment for SCID was stem cell transplantation in which immune cells from a matching donor are transferred to the patient. But it's difficult to find matching donors and, even then, the patient's body may reject the transplanted cells.

    With gene therapy, clinicians remove the patient's own bone marrow, isolate the stem cells, correct the gene and reinsert it into the patient, explained William J. Bowers, associate professor of neurology at the University of Rochester Medical Center in Rochester, N.Y.

    The two current papers detail the success of gene therapy in two groups of patients: 10 boys with X-linked SCID (SCID-X1); and six with ADA-SCID, which involves a slightly different gene mutation. All were between 6 months and 39 months old.

    Gene therapy successfully treated four of the six ADA-SCID patients.

    All the SCID-X1 children recovered, although one developed leukemia. That boy is currently in remission, but leukemia has been a problem with previous gene therapy trials.

    Last year, French researchers reported that eight of nine male infants born with SCID-X1 had recovered as a result of gene therapy. Unfortunately, almost half developed acute leukemia, one of whom died.

    The virus vector used in this earlier trial inadvertently activated an oncogene, which led to the development of the leukemia, researchers said.

    The latest research circumvented this problem by using a different virus vector.

    "A cloud was thrown over the field several years ago and they've solved it nicely," said Dr. Darwin Prockop, director of the Texas A&M Health Science Center College of Medicine Institute for Regenerative Medicine at Scott & White in Temple, Texas. "Very probably this can be used for other genetic diseases."

    "This field came on with huge promise, then hit a few bumps and now . . we're starting to see more and more of these successes," added Bowers.

    Uncovering the spread of deadly cancer: New imaging device enables scientists to see tumor cells traveling in the brain

    ScienceDaily (Aug. 25, 2011) ? For the first time, scientists can see pathways to stop a deadly brain cancer in its tracks. Researchers at Case Western Reserve University School of Medicine have imaged individual cancer cells and the routes they travel as the tumor spreads.

    The researchers used a novel cryo-imaging technique to obtain the unprecedented look at a mouse model of glioblastoma multiforme, a particularly aggressive cancer that has no treatments to stop it from spreading.

    A description of their work, and images, is being published Sept. 1 in the journal Cancer Research.

    "We're able to see things we couldn't before, and we can use these images to understand how tumor cells invade and disperse," said Susann M. Brady-Kalnay, a professor of molecular biology and microbiology at the Case Western Reserve School of Medicine, and senior author of the paper.

    That information, in turn, can be used to help develop and test the effectiveness of drugs and other therapies used to treat the cancer, she said.

    To obtain the view, the scientists used a model that included four different cell lines of brain cancers at various stages of tumor development and dispersion. The cancer cells were modified with fluorescent markers and implanted in the model's brain in collaboration with Biomedical Engineering Professor James Basilion's lab.

    The cryo-imaging system, developed by David Wilson, also a professor of biomedical engineering at Case Western Reserve, disassembles the brain layer by layer and reassembles the model into a color three-dimensional digital image.

    Using software and algorithms designed by the researchers, they are able to differentiate the main tumor mass, the blood vessels that feed the cancer and dispersing cells. The imaging system enables them to peer at single cells and see exactly where they are in the brain.

    The lead researchers, Susan Burden-Gulley, Mohammed Qutaish and Kristin Sullivant, found that two cell lines, a human brain cancer LN229, and a rodent cancer CNS-1, best resemble the actions of glioblastoma multiforme in human patients.

    Reconstructions of models of those two lines enabled the researchers to analyze the extent and patterns of cancer cell migration and dispersal from tumors along blood vessels and white matter tracts within the brain.

    The ability to produce such clear and detailed images, the researchers say, will be invaluable when evaluating the potency of drugs and other therapies designed to block dispersal of glioblastoma multiforme cells.
